Deep in the capitals — Santa Cruz de Tenerife and Las Palmas de Gran Canaria (Las Palmas is on the national short list of large public university hospitals plus private groups) — and thinner in the interior, exactly as the country page says. Medicare pays $0 (Medicare.gov, April 2026); Convenio Especial after year 1, €157/mo per person at 65+.
The springlike south coasts are the product: winter days averaging 21–25°C (November–March, reported Oct 2026), mild evenings — the temperate option while Andalusia bakes and northern Spain cools. Gap: station-level AEMET normals still to be added.
Tenerife is the transatlantic exception: United flies seasonal US nonstops to Tenerife (checked Oct 2026), and Air Canada's Toronto/Montreal services from late October 2026 are reported as the only North America–Canaries nonstops (Aviation Week, Oct 2026). Gran Canaria (LPA): no nonstop found — plan on Madrid or a European hub. Between the islands, ferries and short flights; to the mainland, everything flies.
